This is the print head assembly for the Ultimaker S3 and Ultimaker S5. It includes all pictured parts here, including the printhead cable, PCB, fans, and nozzle seal.

Note: If you have the original version of the S5 (featuring the Ultibot logo on the side panels), we recommend following these instructions to install the printhead without the cable to avoid unnecessary access to high voltage areas.
